BACKGROUND: YouTube is one of the most widely accessed platforms for patients seeking information across all medical specialties. More than 50% of patients engage with online information before seeing a physician. There has been no qualitative analysis of neuromodulation content on YouTube. METHODS: By evaluating the search interest of neuromodulation terms via Google Trends, the authors selected 3 search terms to assess on YouTube. For each term, the top 20 videos were selected, 10 sorting by view count and 10 by relevance. DISCERN criteria were used to evaluate each video. Statistical analysis was performed with R software to discover associations among video characteristics and DISCERN scores (DSs). RESULTS: Neuromodulation, deep brain stimulation, and spinal cord stimulation terms were searched. There were 52 unique and 8 duplicate videos, which were excluded. The videos received 6,984,743 combined views (134,321 average). Neurosurgeons were the most common authors, producing 22 videos, followed by anesthesiologists with 10 videos. Statistical analysis showed that videos that were sponsored (DS 44.79 vs. 34.53; P < 0.011), had board-certified physicians (DS 42.88 vs. 31.25; P < 0.004), and had credentials of the video producer (DS 41.82 vs. 34.42; P < 0.044) were more likely to have higher DSs. Spinal cord stimulation videos had the lowest quality of the search terms (DS 30.9 vs. 39.2, 42.6; P < 0.043). CONCLUSIONS: Certain characteristics of YouTube videos are associated with higher quality. Video producers should aim to use these characteristics and follow DISCERN criteria when preparing online videos.

We investigated the relationships among chronic violence exposure, post-traumatic stress disorder (PTSD) symptom severity, hopelessness, substance use, and perpetuation of violence to facilitate the development of trauma-related interventions for residents of Newark, NJ. A convenience sample of Newark residents (N = 153) was recruited from community centers during various events in 2016-2017. Anonymous, self-report survey measures included a PTSD screen (PCL-C), Beck's Hopelessness Scale, the CAGE questionnaire, and a CDC Health Behavior Scale. Descriptive statistics, Pearson's correlations, Chi square analyses, logistic, and linear regressions were used for analysis. Thirty percent (95% CI [22.7, 37.4]) of our sample screened positive for PTSD. Drug and alcohol use, fighting, and hopelessness were related to severity of PTSD symptoms (p < 0.05). Female gender, CAGE scores, and hopelessness predicted the severity of PTSD symptoms (R2 = 0.354, p < 0.05). Our data has informed the development of a resilience support group currently in the pilot stage for community members.

BACKGROUND: YouTube has rapidly become one of the most widely accessed educational platforms of our time. Over 1 billion hours of YouTube content are viewed every day, including neurosurgical videos. To date, there has never been a qualitative analysis of neurosurgical content on YouTube. METHODS: By evaluating the search interest of neurosurgical terms via Google Trends, we selected 8 search terms to assess on YouTube. For each term, we selected the top 20 videos, 10 when sorting by view count and 10 when sorting by relevance. Using the DISCERN criteria, we evaluated 131 unique YouTube videos to assess bias and overall educational quality. In addition, we categorized each video based on its intended utility and the credentials of the video's commentator. The number of views, average DISCERN scores, and bias scores were compared between search terms and between video categories. RESULTS: There were 131 unique and 29 duplicate videos. The videos received 143,538,363 combined views (1,104,141 average). The average video age was 4.6 years (range, 66 days to 12.5 years). The term "brain surgery" received the most total views (62,339,885), with an average DISCERN score of 1.65 overall and 2 for bias. The average DISCERN score was 2.02 overall and 2.39 for bias. There were 45 advertisements for either hospitals, physicians, or companies, totaling 18,052,873 views and averaging a DISCERN score of 2.04. CONCLUSIONS: YouTube is a frequently viewed but biased source for neurosurgical information. Advertisements are frequently disguised as educational material, and few videos provide references or sources for their information.
